Dr. Falk Pharma announced positive 96-week results from its phase 3 NUC-5 trial of norucholic acid (NCA) for primary sclerosing cholangitis (PSC). NCA demonstrated statistically significant superiority over placebo in achieving the combined primary endpoint of improved liver enzyme levels and no worsening of disease stage. Currently, there are no approved treatments for PSC.
This positive data is a major advancement for PSC patients, who currently face limited treatment options and often progress to severe complications like cirrhosis or liver cancer. The results offer the first real hope for a pharmaceutical intervention that can alter the disease course and improve patient outcomes, potentially delaying or eliminating the need for liver transplantation.
The NUC-5 trial enrolled 301 patients, making it the largest clinical trial conducted on PSC to date, including biopsy comparisons before and after treatment. Analysis at 96 weeks showed 15.1% of NCA patients achieved the primary endpoint compared to 4.2% of placebo patients. Secondary endpoints also favored NCA, with significant improvements observed in liver enzyme levels and disease staging. NCA was well-tolerated, exhibiting a similar safety profile to placebo. The trial will continue for another 96 weeks of double-blind treatment, followed by an open-label extension for patients wishing to continue NCA therapy.
The positive results of the NUC-5 trial signal a potential paradigm shift in PSC treatment. If the remaining trial data confirms these findings, NCA could become the first approved drug for this debilitating disease, offering patients a new standard of care. This breakthrough also opens avenues for further research into the underlying mechanisms of PSC and may pave the way for the development of even more effective therapies.
