A hazard ratio of 0.53 is not a modest signal in a tumor type where post-platinum progression-free survival historically runs under three months — and that is precisely the asymmetry buried inside TEDOVA‘s topline readout. The Phase 2 trial enrolled 185 patients with platinum-sensitive recurrent ovarian cancer who had already exhausted PARP inhibitors and bevacizumab, a population defined by having no remaining standard-of-care options worth the name. In that context, Tedopi® combined with pembrolizumab extended median PFS to 4.1 months versus 2.8 months on best supportive care. The absolute difference looks modest; the hazard ratio does not.

The trial’s 1:1:2 randomization across best supportive care, Tedopi® monotherapy, and the combination arm was a smart design choice because it preserved statistical power for the primary comparison while generating signal on monotherapy activity. That monotherapy arm matters enormously: the HR comparing the combination to Tedopi® alone sat at 0.72, with a p-value of 0.074 — directional but not significant. This means pembrolizumab adds something, but Tedopi® alone is already moving the curve. For a neoantigen vaccine platform repeatedly dismissed as insufficiently active in immunologically cold tumors, that monotherapy signal is the real proof-of-concept claim the investigators are making, and they are not wrong to make it.

The design also forces an honest reckoning with toxicity. Combining a vaccine with anti-PD-1 predictably increased immune-related adverse events, and OSE does not minimize this. The immune-related AE profile will need granular attention when full data are presented at ASCO on May 30 — specifically whether Grade 3+ events were concentrated in the combination arm in ways that constrain patient selection for any Phase 3 iteration. The company’s stated intent is to advance Tedopi® in a Phase 3 in non-small cell lung cancersmall cell lung cancer rather than pivot to ovarian cancer, which means TEDOVA’s primary commercial function is platform validation rather than registration-track data generation.

The single marker to track when Leary presents at ASCO is the subgroup breakdown by HLA type. Tedopi® is an HLA-A2-restricted vaccine, and the enrolled population’s HLA distribution will determine whether the PFS benefit concentrates in a biomarker-defined subset or holds broadly — because that distinction decides whether a Phase 3 in any indication is designed around biomarker enrichment or open enrollment, and those are entirely different development strategies.
