Among the 9 patients per arm who have reached second-look laparoscopy in IMUNON’s Phase 2 MRD study, only 44% treated with IMNN-001 showed residual disease at that surgical checkpoint, compared with 67% in the control group. Those numbers are preliminary and the total target enrollment is 30 patients per arm, but the directional signal matters: the trial was designed to generate translational evidence for a mechanism that has repeatedly failed in the clinic, and so far the biology appears to be cooperating.
The historical problem with IL-12 as an anticancer agent is well established. Systemic delivery produces cytokine release syndrome and dose-limiting liver toxicity severe enough to halt programs entirely. IMUNON’s approach routes around that by encasing an IL-12 DNA plasmid in a nanoparticle and delivering it intraperitoneally, inducing local cytokine expression in macrophages within the peritoneal fluid and tumor tissue rather than flooding systemic circulation. The MRD study has now added bevacizumab to the backbone alongside standard neoadjuvant and adjuvant chemotherapy, and the safety profile remains clean: no cytokine release syndrome, no systemic toxicities, no serious immune-related adverse events. That combination tolerance is a meaningful data point because bevacizumab’s 2018 FDA approval already embedded it deeply in frontline ovarian cancer treatment, meaning any commercially viable regimen will need to coexist with it.
The ctDNA numbers reinforce the MRD finding from a different angle: 87.5% of IMNN-001-treated patients cleared circulating tumor DNA versus 62.5% in the control arm, and 100% of the experimental arm reached no evidence of disease after frontline therapy compared with 56% of controls. These are not the trial’s primary endpoints and the sample is small, but they layer onto the Phase 2 OVATION 2 result that showed a 14.7-month median overall survival gain (45.1 versus 30.4 months). Among patients who also received PARP inhibitor maintenance, a treatment class that now includes multiple FDA-approved agents such as olaparib, the survival separation widened to 24.2 months. That interaction is not accidental: the MRD study assigns maintenance by HRD status, creating a built-in pathway for understanding how IMNN-001 stacks with the standard molecular stratification already in clinical practice.
The single number to watch from here is MRD-positive rate at second-look laparoscopy once the full 15-per-arm cohort completes assessment. If the 44% versus 67% gap holds at final accrual, it supplies the Phase 3 OVATION 3 trial with prospectively validated biomarker logic rather than retrospective correlation, which changes the evidentiary weight of every endpoint that follows.
