Forty percent of wet AMD patients abandon anti-VEGF injections within their first year of treatment, a dropout rate that reflects not efficacy failure but simple exhaustion with the injection calendar. That clinical reality sits at the center of what Ocular Therapeutix is now quantifying more precisely for AXPAXLI: a new post-hoc analysis of the SOL-1 Phase 3 trial estimates a 72% reduction in injection burden over 60 weeks compared with a projected on-label dosing schedule of aflibercept 2 mg every 8 weeks, excluding loading doses. Including loading doses, the reduction is 56%. The math: patients on AXPAXLI averaged an estimated 1.95 injections from Week -8 through Week 52 after a single baseline injection, versus a projected 7.00 for on-label aflibercept. That is not a marginal improvement in a crowded dosing landscape. It is a structural difference in what the therapy asks of patients and caregivers.

The mechanism behind that number is the 66.5% of SOL-1 patients who remained rescue-free under the SOL-R criteria through Week 52. The remaining third who did need rescue averaged just 0.95 additional aflibercept injections, pulling the arm-wide mean down sharply. The post-hoc uses SOL-R’s own rescue threshold of more than 5 ETDRS letter loss and at least 75 microns of central subfield thickness increase, which matters because it pre-empts the obvious methodological objection. Ocular is applying a prospectively defined bar, not a retrospectively softened one. SOL-1 also carries the distinction of being, per the company’s own characterization, the first successful superiority trial of a novel agent against an approved anti-VEGF therapy since the class emerged two decades ago, with AXPAXLI beating aflibercept 2 mg at both Week 36 and Week 52 on vision maintenance.

On the regulatory side, FDA Type C meeting minutes confirmed in May that SOL-1 Week 52 data plus interim SOL-R safety data constitute sufficient evidence for NDA submission. The 505(b)(2) pathway, drawing on axitinib’s decade-plus clinical record in renal cell carcinoma, could shorten review by up to 60 days relative to a standard new molecular entity. A pre-NDA meeting is scheduled for Q3 2026, with the full submission targeting Q4. Meanwhile, Ocular is amending SOL-R to add a superiority comparison against aflibercept 8 mg at Week 96 and a fibrosis and atrophy prevention endpoint, moves that push SOL-R topline results to Q1 2028 but raise the ceiling on what the label could eventually claim. Cash of $598.6 million extends runway into 2028, covering both the submission and that longer confirmatory timeline.

The single variable that will define AXPAXLI’s commercial trajectory before any label expansion data arrives is whether the FDA accepts the 505(b)(2) package as filed and grants the accelerated review clock. If that timeline holds, a 2027 launch becomes a concrete planning assumption rather than a projection, and the adherence crisis documented in real-world AMD populations becomes the primary commercial argument from day one.
