Pulmonary hypertension complicates roughly 83% of HFpEF cases, yet Phase 3 trials in this population have consistently stumbled: the PASSION trial, which tested tadalafil in combined pre- and postcapillary PH with HFpEF, was terminated prematurely. That graveyard of failed programs is precisely what makes Tenax Therapeutics’ topline readout from the Phase 3 LEVEL trial of TNX-103 worth close attention, even before the data are fully public.

Tenax filed an 8-K announcing topline results from LEVEL and scheduled a conference call for August 10, 2026. The filing is short on numbers, which is itself a signal worth reading carefully. Companies with clean, positive pivotal data rarely wait for a scheduled webcast to say so plainly. The controlled disclosure suggests management wants a structured presentation context, whether because the results require nuanced interpretation, because the primary endpoint story is mixed, or because secondary endpoints need to carry weight. TNX-103 is an oral formulation of levosimendan, a potassium-ATP channel activator and calcium sensitizer with a mechanism distinct from the vasodilator approaches that have historically failed in postcapillary PH. That differentiated pharmacology was the scientific rationale for running a Phase 3 at all.

PH-HFpEF remains a condition where treatment options are limited and where the hemodynamic complexity, elevated pulmonary pressures driven primarily by left heart dysfunction rather than intrinsic vascular disease, has defeated drugs that work well in other PH subtypes. Levosimendan’s proposed ability to reduce right ventricular afterload while improving myocardial contractility without raising oxygen demand gave it a plausible fit. Whether LEVEL’s design and patient population were sensitive enough to detect a meaningful signal is the question the August 10 data drop will have to answer. The trial’s primary endpoint and the magnitude of any effect on exercise capacity or hemodynamic parameters will determine whether the mechanism translates into regulatory traction.

The single number to watch when the full presentation lands is the change in pulmonary vascular resistance or a surrogate hemodynamic endpoint: if TNX-103 moved that marker cleanly, a regulatory path exists regardless of what happened to functional endpoints. If it did not, the mechanistic story dissolves with it.
