Most Alzheimer’s drug programs chase amyloid or tau in patients who already have symptoms. NewAmsterdam Pharma is going earlier: the company has dosed its first participant in SPINOZA, a Phase 2b trial testing obicetrapib in adults who carry biomarker evidence suggestive of preclinical Alzheimer’s disease but have no noticeable memory or functional symptoms yet. That population sits upstream of where approved anti-amyloid therapies such as lecanemab currently operate.
Obicetrapib is a CETP inhibitor, a mechanism built to raise HDL and lower LDL rather than clear amyloid plaques. The cardiovascular rationale for the molecule is well-established: in the Phase 2 ROSE trial, the 10 mg oral dose cut median LDL-C by 51 percent on top of high-intensity statin therapy. What NewAmsterdam is now testing is whether that same lipid biology translates into a meaningful signal in a brain disease context, at a stage where cognitive decline has not yet begun. That is a large assumption, and SPINOZA is the study designed to stress-test it.
The scientific logic connecting CETP inhibition to neurodegeneration runs through cholesterol metabolism in the central nervous system and observational associations between HDL function and dementia risk, but the mechanistic chain is not settled. Enrolling people without symptoms adds a second layer of difficulty: measuring a treatment effect before decline is apparent demands either very sensitive biomarker endpoints or a very long follow-up, and often both. The trial design will determine whether SPINOZA can actually detect a signal, or whether any effect gets lost in the noise of a population that, by definition, looks clinically healthy at baseline.
If obicetrapib produces a biomarker or cognitive signal here, it would be the strongest evidence yet that CETP inhibition has a role outside cardiovascular medicine. The number to watch when data emerge is not a clinical scale score but the biomarker trajectory in the preclinical subgroup, since that is what will determine whether a larger prevention trial is defensible.
