Zenas BioPharma released new Phase 3 data on obexelimab in IgG4-related disease on October 12, 2026, including results from the open-label extension of its pivotal trial. The open-label extension data matters because it speaks to durability: short-term remission in IgG4-RD is achievable with steroids, but the disease relapses in roughly half of patients, and whether obexelimab holds its effect over months rather than weeks is the question that will shape its commercial and regulatory path.

The competitive context is real. Inebilizumab (UPLIZNA) received FDA approval for IgG4-RD in April 2025, so obexelimab enters a market that already has a CD19-targeting approved therapy. Where obexelimab differs mechanistically is its non-cytolytic profile: rather than depleting B cells, it co-engages CD19 and the inhibitory receptor FcγRIIb, effectively quieting B-cell activity without killing the cells. Whether that distinction translates to a cleaner long-term safety profile or meaningfully different relapse rates in the extension data is what the full press release should clarify.

IgG4-RD affects roughly 5.3 per 100,000 people in the United States, with a mortality rate about twice that of the general population, so the patient population is small but the disease burden is serious. Zenas disclosed the data via an 8-K Regulation FD filing, attaching the press release as Exhibit 99.1 rather than summarizing the results in the body of the form. That means the numbers themselves are not in the public SEC text as of the filing, and investors and clinicians will need the exhibit to evaluate the actual flare rates and safety signals from the extension period.

The number to watch from the full release is the flare rate in patients who continued obexelimab into the open-label extension versus those who completed the blinded period. If the extension shows sustained disease control past 12 months, it gives Zenas a durability argument that could matter both to FDA reviewers and to rheumatologists already familiar with inebilizumab’s profile.
