Shionogi & Co., Ltd. has released new data from PROVE, a large-scale global study examining real-world evidence for Fetroja®/Fetcroja® (cefiderocol). Cefiderocol is a novel antibiotic used to treat severe infections caused by Gram-negative bacteria, particularly those resistant to carbapenems, in adults.

The PROVE study, a retrospective review of medical charts, encompassed 1,075 hospitalized patients treated with cefiderocol for Gram-negative bacterial infections between 2020 and 2024. The study aimed to understand patient characteristics, treatment outcomes, and cefiderocol’s effectiveness in real-world clinical settings. Results showed a favorable clinical response in 75.1% of patients at the end of treatment, indicating the resolution or improvement of infection symptoms.

The study included a diverse patient population, with the majority (56.6%) receiving treatment in intensive care units. Respiratory tract infections (53.1%) were the most common, followed by urinary tract infections (10.6%) and bloodstream infections (10%). Significantly, 74.6% of the infections demonstrated resistance to carbapenems, highlighting the severity and challenging nature of these infections. The most common pathogens were Pseudomonas aeruginosa, Acinetobacter baumannii, and Enterobacterales. Clinical response rates were promising across different infection types.

Data presented at IDWeek 2024 emphasized the in vitro activity of cefiderocol against resistant bacteria. Studies revealed that cefiderocol remained effective against multidrug-resistant strains, including those resistant to multiple beta-lactam/beta-lactamase inhibitor combinations. Notably, cefiderocol exhibited potent activity against bacteria producing metallo-beta-lactamases (MBLs), enzymes that render most antibiotics ineffective.
