About 12 percent of stage III colon cancers carry a deficient DNA mismatch repair (dMMR) signature, a mutation profile that leaves tumors unusually exposed to immune attack. The FDA’s approval Friday of atezolizumab (Tecentriq and its subcutaneous formulation Tecentriq Hybreza) added to standard fluoropyrimidine-oxaliplatin chemotherapy in that specific setting gives oncologists their first checkpoint inhibitor option in the adjuvant colon cancer space, where the goal is eliminating residual disease after surgery rather than controlling metastatic spread.
The approval rests on data from the ATOMIC trial (Alliance A021502), which demonstrated that layering atezolizumab onto the mFOLFOX6 backbone significantly improved disease-free survival in dMMR stage III patients. That chemotherapy platform has been adjuvant standard of care for two decades; the Roche approval grafts immunotherapy on top of it for a molecularly defined subset. This is the twelfth U.S. indication for Tecentriq.
The dMMR/MSI-H distinction matters for context. Pembrolizumab earned FDA approval in 2020 for first-line treatment of unresectable or metastatic MSI-H/dMMR colorectal cancer, based on KEYNOTE-177. Atezolizumab’s approval occupies a different clinical slot: resected stage III disease, adjuvant intent, earlier in the treatment continuum. The two approvals do not directly compete for the same patient, though any patient who recurs after adjuvant atezolizumab will raise sequencing questions for treating physicians about subsequent PD-1/PD-L1 exposure.
For trial teams running adjuvant colorectal studies, the ATOMIC readout tightens the comparator conversation. Any future adjuvant trial in dMMR stage III colon cancer now has to account for an approved regimen that includes atezolizumab. Protocol teams designing control arms or enrollment criteria in this population will need to address that directly in their trial designs.
