NKGen Biotech has received FDA authorization for an Expanded Access Program (EAP) to explore the use of its cryopreserved, autologous NK cell therapy, troculeucel, in various neurodegenerative diseases. This EAP builds on promising Phase I results in Alzheimer’s disease, where troculeucel showed clinical benefit without drug-related adverse events, and expands the investigation to include Parkinson’s Disease, ALS, and other less common neurodegenerative conditions. The FDA has authorized enrollment of up to 20 patients, with anticipated enrollment commencing in Q3 2025.

This EAP is a crucial step for patients suffering from neurodegenerative diseases, many of whom currently lack effective treatment options. The expansion to other conditions beyond Alzheimer’s, based on the shared characteristic of autoimmune neuroinflammation, offers a potential new avenue for therapeutic intervention in these devastating diseases. The preliminary success of troculeucel in crossing the blood-brain barrier and reducing neuroinflammatory markers suggests a mechanism that could be applicable across multiple neurodegenerative conditions. This provides a much-needed glimmer of hope for patients facing limited therapeutic choices.

The EAP for troculeucel will be an open-label, non-randomized, multi-center study, allowing for a broader assessment of the therapy’s safety and efficacy across a range of neurodegenerative diseases. Although currently focusing on a Phase 2a trial in moderate-stage Alzheimer’s, this EAP allows NKGen to gather valuable real-world data in a wider patient population and potentially identify additional indications for troculeucel. The speed of patient enrollment is contingent on securing the necessary funding.

This EAP represents a significant advancement in the development of troculeucel. Positive outcomes could lead to broader clinical trials and eventually, regulatory approval for various neurodegenerative diseases. The data generated from this program will be critical in informing future development strategies and potentially accelerating the path towards providing a much-needed new treatment option for patients with these challenging conditions. The progress of this EAP and securing additional funding will be key factors determining the future trajectory of troculeucel and its impact on the landscape of neurodegenerative disease treatment.
