Castle Biosciences presented new research on cutaneous and uveal melanoma (CM and UM) at the 2025 American Association for Cancer Research Annual Meeting. The research focuses on using gene expression profiles to improve risk stratification and guide personalized treatment strategies for these cancers. The goal is to improve patient outcomes through earlier detection of metastasis and tailored surveillance.

This research is vital for melanoma patients as it addresses critical unmet needs. For CM, the research builds upon previous findings, demonstrating the ability of the DecisionDx-Melanoma test to identify early-stage patients at high risk of metastasis to various sites, including the central nervous system, lung, liver, and bone. This allows for more informed surveillance strategies and potentially earlier interventions. For UM, the research explores using aqueous humor protein biomarkers as a less invasive way to assess the malignant potential of small uveal melanocytic tumors. This is especially important since repeated biopsies of eye tumors are not feasible.

The CM study showed a significantly higher rate of distant metastasis in patients with Class 2B (high risk) DecisionDx-Melanoma test results compared to Class 1A (low risk) across multiple metastasis sites. This difference persisted for several years post-diagnosis, particularly for CNS and lung metastasis. The UM study found significant protein differences in the aqueous humor between high-risk and low-risk UM patients as assessed by the DecisionDx-UM test. This could lead to the development of new risk prediction models for UM using less invasive liquid biopsies.

This research strengthens the potential of gene expression profiling and protein biomarkers to personalize melanoma care.
