Zhimeng Biopharma enrolled its 255th patient into the pivotal Phase 2/3 trial of CB03-154 for ALS, completing recruitment across 15 centers in mainland China. The number itself is the first piece of news worth watching: 255 patients in an ALS study conducted entirely in China means the readout, targeted for Q3 2027, will carry real statistical weight while being shaped by a patient population where most global ALS approvals have limited data. If Phase 2 topline results are positive, the company plans to file for conditional approval from China’s CDE without waiting for the full Phase 3 dataset.

The primary endpoint is change from baseline on the ALSFRS-R score at Week 39. That functional rating scale is the standard measure in ALS trials, and a 39-week read is long enough to capture meaningful decline trajectories in a disease that progresses at roughly two to four ALSFRS-R points per month in untreated patients. CB03-154 is a selective opener of voltage-gated KCNQ2/3 potassium channels, a mechanism designed to modulate neuronal excitability and restore the balance between excitation and inhibition in motor neurons. Ezogabine, an earlier KCNQ2/3 opener, reduced pathological excitability in ALS motor neurons in a prior study, though it was never developed for ALS commercially. Zhimeng’s compound is positioned as a more selective next-generation version, though preclinical neuroprotection data still needs clinical validation in this trial.

The regulatory setup adds a layer worth understanding. CB03-154 received U.S. FDA Orphan Drug Designation in October 2023, but the current study is China-only, led by Professor Fan Dongsheng of Peking University’s Third Affiliated Hospital. China’s CDE conditional approval pathway allows marketing authorization based on early evidence of benefit in serious diseases before a full confirmatory dataset is complete, which is why a Q3 2027 Phase 2 read could translate directly into a filing. Approved options for ALS remain limited globally, with drugs like riluzole extending survival by a modest average of roughly three months in initial studies, leaving substantial room for a therapy that slows functional decline more meaningfully.

CB03-154 is also in a Phase 2 trial for focal epilepsy in China and Australia, and the CDE cleared a Phase 2 start in major depressive disorder in June 2026. The ALS readout in mid-2027 represents the most advanced of these programs by enrollment status,, and it will test whether the KCNQ2/3 mechanism translates from preclinical motor neuron models into a measurable functional benefit for patients over nine months.
