Anthos Therapeutics presented new analyses from the AZALEA-TIMI 71 study at the American College of Cardiology Annual Scientific Session. The analyses revealed that abelacimab, a Factor XI inhibitor, significantly reduced bleeding in patients with atrial fibrillation, irrespective of age or pre-existing bleeding risk, compared to rivaroxaban. The data showed a greater absolute risk reduction in bleeding for patients older than 75 and those with higher baseline bleeding risk.

These findings are potentially groundbreaking for managing atrial fibrillation, a condition often requiring anticoagulants to prevent stroke but carrying significant bleeding risks, particularly in older populations and those with multiple health issues. A safer anticoagulant could allow more patients, especially those currently undertreated due to bleeding concerns, to receive appropriate therapy, minimizing stroke risk without significantly increasing bleeding risk.

The first analysis showed a 6.2% absolute risk reduction in major or clinically relevant non-major bleeding for patients 75 and older taking abelacimab compared to 4.2% for those younger than 75 taking the same drug. The second analysis, which stratified patients by bleeding risk using the DOAC score, demonstrated increasing absolute risk reduction in bleeding with abelacimab as baseline risk increased. Patients deemed very high risk saw a 14.1% absolute risk reduction with abelacimab versus rivaroxaban.

These results hold significant promise for the future of anticoagulation therapy. If approved, abelacimab could offer a safer alternative to current treatments, particularly for higher-risk patients, potentially changing clinical practice and improving patient outcomes. The ongoing Phase 3 trials (LILAC-TIMI 76, ASTER, and MAGNOLIA) will provide further insights into abelacimab’s efficacy and safety profile, paving the way for its potential use in diverse patient populations.
