SFJ Pharmaceuticals and SERB Pharmaceuticals announced positive final results from the Phase 3 REVERSE-IT trial of bentracimab, a ticagrelor reversal agent. The trial met its primary endpoint, showing rapid restoration of platelet function in patients requiring urgent surgery or experiencing major bleeding. This was achieved within 5-10 minutes of bentracimab administration and maintained throughout the infusion period and beyond. The trial also achieved its secondary efficacy endpoint with the majority of eligible patients achieving effective hemostasis within 24 hours.
This development is crucial for patients on ticagrelor, a powerful antiplatelet medication, who require urgent surgery or experience major bleeding. Currently, discontinuing ticagrelor and waiting for its effects to dissipate can take several days, posing significant risks. Bentracimab offers a rapid solution to restore platelet function, enabling timely intervention and potentially saving lives. This advancement significantly improves the risk-benefit profile of ticagrelor, expanding its potential use in broader patient populations.
The Phase 3 trial enrolled 226 patients and evaluated a specific bentracimab dosing regimen involving a bolus followed by loading and maintenance infusions. The drug demonstrated a favorable safety profile, with no serious adverse reactions or allergic reactions observed. The rate of thrombotic events was consistent with the expected baseline for the patients’ underlying conditions. Importantly, ticagrelor or other P2Y12 inhibitors could be re-initiated within a week after bentracimab infusion, allowing patients to regain the benefits of antiplatelet therapy.
Bentracimab’s rapid onset and relatively short half-life provide a critical tool for managing the delicate balance between bleeding and thrombosis risks. The ability to quickly reverse ticagrelor’s effects and subsequently reinstate its benefits enhances the drug’s clinical utility and addresses a significant unmet medical need. This positions bentracimab to become a standard of care in managing ticagrelor-treated patients requiring urgent procedures or experiencing major bleeding episodes, ultimately improving patient outcomes.
