Tectonic Therapeutic announced complete results from Part A of its Phase 1b clinical trial of TX45 for Group 2 Pulmonary Hypertension in Heart Failure with preserved Ejection Fraction (PH-HFpEF). The data, presented at the European Society of Cardiology Heart Failure 2025 Congress, confirmed TX45’s tolerability and demonstrated improvements in left ventricular function and pulmonary hemodynamics. New echocardiographic analysis revealed sustained hemodynamic effects for 29 days following a single intravenous dose.
This data is particularly important because PH-HFpEF, a condition with no approved treatments, impacts a significant patient population. The sustained hemodynamic improvements observed with TX45 suggest its potential as a long-acting therapeutic option, which could significantly enhance patient care. The positive results across a range of left ventricular ejection fractions (LVEF) further expand TX45’s potential applicability to a broader spectrum of PH-HFpEF patients.
The complete Part A results of the Phase 1b trial demonstrated a 19% reduction in pulmonary capillary wedge pressure (PCWP) in the overall patient population. For the subgroup with combined pre- and post-capillary pulmonary hypertension (CpcPH), TX45 demonstrated a greater than 30% reduction in pulmonary vascular resistance (PVR). These endpoints correlate with improved exercise capacity, morbidity, and mortality. Furthermore, sustained improvements in echocardiogram endpoints, including increased tricuspid annular plane systolic excursion/systolic pulmonary artery pressure (TAPSE/SPAP) and right ventricular fractional area of change (RVFAC), were observed for 29 days post-treatment. Importantly, TX45 was well-tolerated, with no serious or severe adverse events reported.
These positive findings from the Phase 1b trial support further investigation of TX45 as a potential best-in-class therapy for Group 2 pulmonary hypertension. The data underscore the potential of TX45 to address the significant unmet medical need in this patient population. The anticipated results from Part B of the Phase 1b study in patients with pulmonary hypertension in heart failure with reduced ejection fraction (PH-HFrEF), expected in the second half of 2025, along with the ongoing APEX Phase 2 clinical trial with topline results expected in 2026, will be crucial in shaping the future development and potential market entry of TX45. These upcoming milestones have the potential to establish TX45 as a transformative treatment option for various forms of pulmonary hypertension associated with heart failure.
