Tango Therapeutics has initiated a Phase 1/2 clinical trial of TNG456, a brain-penetrant PRMT5 inhibitor, for patients with MTAP-deleted solid tumors, particularly glioblastoma (GBM). The trial will evaluate TNG456 as a monotherapy and in combination with abemaciclib, focusing initially on GBM patients in the monotherapy dose escalation phase. This trial represents Tango’s commitment to developing targeted cancer therapies based on the principle of synthetic lethality.

This development is particularly crucial given the limited treatment options and dismal prognosis for GBM patients, who face a five-year survival rate below 10%. With MTAP deletions present in 45% of GBM cases, TNG456 offers a potential therapeutic avenue for a significant portion of this patient population. Preclinical data suggest the drug’s potency, MTAP-selectivity, and ability to penetrate the blood-brain barrier, raising hopes for clinically meaningful efficacy in GBM.

The Phase 1/2 trial (NCT06810544) will assess the safety, pharmacokinetics, pharmacodynamics, and anti-tumor activity of TNG456. The initial focus is on dose escalation with TNG456 monotherapy in GBM patients, followed by exploration of combination therapy with abemaciclib.

The initiation of this trial represents a significant milestone in the development of targeted therapies for GBM. Positive clinical data could lead to a much-needed new treatment option for patients with this aggressive cancer, potentially altering the treatment landscape and improving patient outcomes. The trial’s progress and results will be closely watched by the oncology community as an indicator of the potential of MTAP-targeted therapies.
